THE OPPORTUNITY
Deliver community-based primary care with an emphasis on underserved patients. Address preventive care, chronic conditions and social factors affecting health, maintain accurate electronic records, and coordinate laboratory, imaging and referral follow-up.
SCHEDULE AND TEAM
Detailed notes describe a typical four-day full-time workweek with 20% administrative time, with some three-day arrangements. Productivity is described as at least 18 billable encounters per day, adjusted for patient acuity, visit type and setting. Collaborate through staff meetings, peer review and case conferences.
HOUSING, RELOCATION AND SUPPORT
